A user/person inputting/reporting a maintenance item is not necessarily the owner/inspector on the report.
i.e. maintenance guy Mark Maint reports that a bridge rail needs to be repaired, Patty Pothole reports that there's a pothole on a bridge, Wanda Waters reports that there is water leaking from the bridge above her onto cars below, a called in report of debris on the roadway, or inspector Bill Bridges reports that he found delamination on the bridge deck
Because the inspector name/Team Leader name is displayed in the header of all maintenance items, auditors mistakenly believe that the inspector/Team Leader (Bill Bridges) is the one reporting the issue. In actuality, it is one of the other people listed above (Mark Maint, Patty Pothole, Wanda Waters, called in report) who should be listed as the "maintenance reported by XXX or YYY" in the photo header
